How to Apply Transdermal Estrogen (Estradiol) and Progesterone Cream

Transdermal hormone creams, such as estradiol and progesterone, are commonly prescribed for hormone balance, menopausal support, or specific medical needs. Using them correctly is important to ensure proper absorption and effectiveness. Here’s a simple guide to help you apply the cream safely and consistently.

 

Dispensing the Cream

  • Gently press the pump all the way down to dispense the exact amount prescribed.
  • Apply the cream to clean skin on the inner arms, inner thighs, or abdomen.

Spreading the Cream

  • Use your inner arms to spread the cream evenly across the application area.
  • Do not rub too hard — a gentle spread is enough.

Important Reminders

  • Avoid applying the cream to your breasts or inside the vagina, unless specifically instructed by your doctor.
  • After application, wait at least 2 hours before skin-to-skin contact with others to prevent accidental transfer.

 

When to Apply the Cream (Cycle-Based Use)

  • Start using the cream on Day 5 of your menstrual cycle.
  • Continue applying it daily until Day 24.

If your period starts early (before Day 24):

  • Stop using the cream.
  • Restart on Day 5 of your next cycle.

If you notice spotting before Day 24:

  • Keep using the cream.
  • If the spotting turns into a full period, stop and restart on Day 5 of your next cycle.

If you don’t get your period after Day 24:

  • Wait 5 days.
  • If your period starts, restart on Day 5 as usual.
  • If there’s still no period after 5 days, restart the cream anyway and continue for 20 days.
  • Always make sure you are not pregnant. Take a urine test if you’re unsure.

 

Special Instructions (Only If Advised by Your Doctor)

Some women may be instructed to use the cream daily regardless of their cycle.
In rare cases, your doctor may suggest a combined schedule:

  • Apply estradiol from Day 5 to Day 24.
  • Add progesterone from Day 15 to Day 24.

This approach is more complex and should only be followed if recommended by your healthcare provider.
